MediCal –  Requirements:

  1. ID – Birth certificate, driver’s license, passport, etc.
  2. Proof of income – Wage stubs for last two months
  3. EDD Benefits – Unemployment or state disability insurance
  4. Bank statements – for all accounts
  5. Vehicle registration
  6. Rent or mortgage verification
  7. Proof of pregnancy – if applicable

County Medically Indigent Service Program (CMSP) /
Sliding Fee Scale – Requirements:

  1. Proof of Income – Wage stubs, Social Security Award letter, Retirement Benefit Award Letter, Pension, VA, Bank Statements, Prior Year Income Tax Documentation, Student Loan letter
  2. Proof of Identification – California DMV License or DMV ID card, or Passport
  3. Proof of Residency – Recent rent/mortgage receipt, current motor vehicle registration, current / recent utility bill

Family PACT Requirements:

  1. Resident of California
  2. Family income at or below 200% federal poverty level
  3. At risk for pregnancy (men and women)
  4. NO INSURANCE including MediCal, Emergency MediCal, Share of cost MediCal, Insurance but has not met deductible.  (ONLY EXCEPTION – has insurance, but must keep family planning services confidential)
  5. Age limit is 55 for women, 60 for men

Pregnant, sterilized, or menopausal clients are not eligible for Family PACT services
