Background

AmeriCorps is the national service program that engages Americans of all ages and backgrounds in service in the five priority areas of education, public safety, environment, homeland security, and other human needs. AmeriCorps service gives their members the opportunity to impact change in underserved communities, gain valuable life experience, and take steps toward their own future goals.

LifeLong AmeriCorps program mission and vision is to become a pipeline for careers in community health centers that is improving access to primary and preventative care services for the medically underserved communities that we serve.

Positions/Activities

The AmeriCorps member will serving side-by-side with health center staff, you have the opportunity to be of service to your community and to learn about health careers. Members will provide culturally and linguistically appropriate health education, outreach, referrals and enrollment into programs directly to community members while getting the skills they need to pursue a health profession.

Member position will compose of:

Health Education
Health Coaching
Care Coordination
Case Management
Patient Navigation
Community Engagement & Outreach

Member activities will include

Improve Utilization of Health Services and Programs: Provide health education in one-on-one or group settings; assist individuals with navigating health related services/programs; transportation assistance; provide positive reinforcement of treatment plans and prevention goals; follow-up as needed.

Enrollment: Assist individuals with the enrollment process for health programs and health insurance plans.

 

Outreach and Advocacy: Conduct outreach in the community (i.e. schools, community centers, events, and shelters); provide education about available health resources and related programs, and health care options.

Volunteer Recruitment and Volunteer Training/Management: Assist with recruitment, training, and retention of non-AmeriCorps volunteers; assist with the management of non-AmeriCorps volunteers.

Service Projects: participate in various community service projects; coordinate and engage members in planning and participating in community service projects.
On-going Training: on health skills and resources as needed for placement, plus professional development training as available.

Recruitment Information

The application for the 2017-2018 Program Year will begin on Friday, July 14, 2017. All applications are due by Monday, August 14, 2017.

The Service Start Date will be Monday, September 18, 2017.

August 15, 2017 Primary and secondary interviews for qualified applicants begin
August 25, 2107 Primary and secondary interviews for qualified applicants end
  Applicants will be notified of their final application status during the interview process
September 12, 2017 Service Start Date: The 2017-18 AmeriCorps program year begins
July 20, 2018 Service End Date: The 2017-18 AmeriCorps program year ends

 Applications will be reviewed on a rolling basis.

 

 

How To Apply

We are currently recruiting for our 2017-2018 program year and will be filling 20 positions.

Term of Service

Members serve a minimum of 1,700 service hours over a 10 to 12 month period.

Qualifications

Applicants must be 18 years of age, or at least 17 years old with the signature of a legal guardian. Applicants must have a high school diploma, GED or commitment to obtain during their service year. Applicants must be a U.S. citizen, national or lawful permanent resident.

Applicants must make a strong commitment to service, respect and interest in working in medically underserved communities, and demonstrate abilities to work with diverse populations.

1. Register on My AmeriCorps

Click here  to create your own personal My AmeriCorps account. My AmeriCorps is an online system designed to help members manage their AmeriCorps experience. It provides an easy way for individuals to get and give important information such as view the status of their AmeriCorps application, update profile information and references, search for opportunities of interest, and submit completed applications.

2. Submit Your Completed My AmeriCorps Application

Submit your completed My AmeriCorps application via your My AmeriCorps account to “LifeLong Medical Care/East Bay Area.” We will access your application through the My AmeriCorps portal and you will be notified when it is under review.

3. Submit and Completed Our Supplemental Application and Resume

Click here to access LifeLong Medical Care’s online AmeriCorps Supplemental Application. Complete all required fields in the Supplemental Application, and upload your personal statements and resume in the link above.
