Introducing Patient Portal/MyChart, a service for LifeLong patients that connects you directly to your health information and much more! In addition to your personal health information, Patient Portal/MyChart allows you to do things such as make appointments, renew prescriptions, and make other requests directly through an online portal. All of these features are available at your convenience, rather than having to call on the phone or come into the health center. You can access Patient Portal/MyChart through your computer, tablet, or smart phone.

In an effort to improve access and better address your medical needs, we have expanded health center hours to include more nights and weekends.
If you would like to speak to a provider to help you decide how to treat an illness after hours or to help decide whether to obtain urgent care, please call (510)981-4100.
Along the same lines, please notify your provider or Patient Services within 48 hours of receiving care at a hospital emergency room or urgent care center. This will allow us to schedule an appropriate follow-up appointment for your care needs.
