LifeLong Medical Care provides high-quality medical, dental, and social services to people of all ages. We are dedicated to helping each patient live a healthy productive life. In addition to offering care, we offer a range of patient education and outreach services.
LifeLong Trust Health Center serves adults with disabling conditions and histories of homelessness who also participate in Alameda County’s General Assistance Program.
The mission of LifeLong Trust Health Center is to improve the lives of clients by providing high-value care that addresses clients’ physical, mental, and social well-being in one location. This collaboration between multiple government agencies and non-profit organizations will promote dignity, respect, and compassion in all interactions among staff and clients. Participating organizations will collaborate closely to support clients to find and maintain stable homes, to promote their overall health and well-being, and to develop or re-establish strong social support networks outside of the public services system.
